as a compulsory posting; i was made to electively plan a posting for one month..
the perfect choice should be a place where i don't have to spent much money for good meals, with least transportation cost... a.k.a. closest to home.
here i am; the first day of my 2 week attachment at a district hospital.
frankly speaking, my expectation lacks 70% energy expenditure.
believe it or not; only in the morning, i was made to do 2 discharge summary and clerk two patients plus writing the plans, yadda2.. in A & E and the works continue in the afternoon. but that's fair..
i mean believe it or not.. while clerking my first patient. a staff came to me and say: " is this ECG ok dr?".. i was like.." i think it's OK".. and later he asked me " should we just discharge him?".. and i go.. "uh -oh... i'm a medical student. i dont think i'm supposed to give the decision".. and he went away. he later come back with other patient summary and investigation detail and said " dr, what should we do now? " and i said. " erm.. i don't know.. i think we should wait for the dcotor to decide" .. and he goes " never mind, could you just 'cop' here" and i smile back " i don't have a 'cop' since i'm still a student"..
later,
" bla2.. eh cik, encik accident kat mane?" ( me clerking the MVA patient)
and a hospital staff came with an ECG of another patient.. " Dr. please review this ECG and what we should do next?"
and i goes "haha.. nanti saya tanya Dr **** ye.."
and he replied : " saya dah call Dr ****, dan dia pesan, kalau saya cakap kalau saya tak nak bagi decision, cakap balik kat anum jangan nak ngada2. bagi je decision"..
and i goes " hahaha.. " sambil memandang dengan ikhlas ke arah hospital staff tersebut.
blank.
" what is it?" ." It's an ECG"
i stared at the ECG for around 1 minute.
" I think there's ST elevation at lead #"
and the staff said; " yep, but it's just one lead Dr, others look OK. so what should we do then?"
i said " what do you suggest?"
" i'll go for cardiac enzyme".
and i say " yup. Cardiac enzyme. thank you"
and the staff said " Ok Dr"..
and i looked at the patient's face without blinking for a while.
then a staff came again " owh. when would i finish clerking.."
" Dr, a young female student came for shortness of breath ... yaddda....."
and i asked him:
" how about her ABG?"
" Dr, we don't do ABG here"
"Really?"
"Yep"
and then he goes " Ok.. we'll discharge her right?"
i stared back at him and said : " Do what you think is right. and i'll learn from you"
and he looked at me in a weird way..
haha.
in the evening.. i fortunately have a comfroetable discussion with the new busy director..
" i think you should do on call maybe 2 to three times here, stay in the hospital"
me " hehe. ok. no problem". smile.
and really! i learnt a lot. people shouted at me the first day because i don't now the exact definition of hyperemesis gravidarum " excessive vomiting during pregnancy... elevated B-HCG?"... " what? your definition was like...(fortunately i couldn't remember the uneasy thing)" and the whole ward is staring at me.. that;s in my first hour in the ward. khekeh..
anyhow. i planned to learn more, and grew older.
